Family Health Network

Family Health Network (FHN) began in 1995. We had an important goal. It was to bring quality health insurance to people who need it the most. FHN is a not-for-profit provider sponsored HMO. It is directed by local health care providers. We serve people living in the Chicago area. This includes Cook, DuPage, Kane, Kankakee, Lake and Will Counties. FHN also serves people in the Rockford area. This includes Boone, McHenry and Winnebago Counties. For 20 years, we have made strong relationships with medical providers. That has helped us give more medical care to the people we serve. As we grew, we have become a leader in Medicaid. Medicaid is a program sponsored by the government. We serve our members with great providers and hospitals. We also offer special centers for mental health and children`s health. FHN is successful because we give our members quality care. We think of every provider as a partner, not just a paid contractor.

Chambliss Center for Children

Chambliss Center for Children is a private, local non-profit organization in Chattanooga, Tennessee. Dating back to 1872, the agency exists to preserve family unity and to help prevent the dependency, neglect, abuse, and delinquency of children by responding to the communitys child care needs. We accomplish this through two major programs: 1. Extended Child Care: We offer early childhood education and child care to children ages 6 weeks through 12 years of age. Parents must be working or in school to use our program, but their fees are based on income, making it affordable to everyone. Our main campus is open 24 hours per day, weekends, and holidays, to allow parents to work any hours or shifts needed. A Three Star rating from the State gives us the highest quality program possible. We have five off-site child care programs, operating similar programs, without being open 24 hours. 2. Residential Care: We care for children who have been removed from their homes and are in the custody of the State of Tennessee. They have been abused, abandoned, neglected, or come through the juvenile court system. Children are primarily ages 6-18 and can either reside in our Shelter or in our foster homes throughout the city. The goal is to reunite the children with their families or to find a suitable adoptive placement. Our programs serve almost 700 children per day, of all races, ages, and economic backgrounds throughout the city.
