Thresholds is the oldest and largest provider of recovery services for persons with mental illnesses in Illinois. We offer 25 innovative programs at more than 70 locations throughout Chicago, the adjacent suburbs, and McHenry and Kankakee Counties. Services include assertive outreach, case management, housing, employment, education, psychiatry, primary care, substance abuse treatment, and pioneering research at the Thresholds-Dartmouth Research and Evaluation Center. Last year, Thresholds served 6,100 adults and youth, with 75% of services delivered out in the community, representing more than 400,000 hours of care.

National Organization for Rare Disorders

The National Organization for Rare Disorders, a 501(c)(3) organization, is an independent patient advocacy organization dedicated to helping individuals with rare diseases and the organizations that serve them. NORD, along with its 280 patient organization members, is committed to the identification, treatment, and cure of rare disorders through programs of education, advocacy, research, and patient support services. NORD serves all stakeholders in the rare disease community, including patients and their families, patient organizations, researchers, medical professionals, medical students, and companies developing orphan products. NORD also works closely with many government agencies, most notably the National Institutes of Health (NIH) and the Food and Drug Administration (FDA). All NORD programs are focused on one ultimate goal -- to improve the lives of individuals and families affected by rare diseases. NORD is the official sponsor of Rare Disease Day in the U.S., an observance day held on the last day of February each year worldwide. Its goals are to raise awareness for rare diseases and improve access to treatment and medical representation for individuals with rare diseases and their families.

Monarch

Monarch is a nonprofit organization that supports thousands of people statewide with intellectual and developmental disabilities, mental illness, and substance use disorders. The agency provides services and supports to people of all ages, skill levels, and backgrounds. We do not make decisions for the people we support. Monarch professionals, like our behavioral health services counselors and direct support professionals, will work with individuals to discover their interests and help them to identify their place in the community where they live.
