We provide access to high-quality, equitable, healthcare to everyone in our community. CCHS is a nationally recognized Primary Care Medical Home, delivering higher quality and more integrated care than many health systems throughout the United States. In September 2019, Choptank Community Health was awarded a Gold-level award by the American Heart Association for improving the health and well-being of patients with high blood pressure.
