We collectively create health or lack of health. Caregiver, service giver, policymaker and, most importantly, each one of us. Healthy individuals can create a healthy community. We are all in this together, creating, delivering and receiving care and services. Our CCO is a single organism, a community, an integrated collection of care and services that prevents illness and promotes and restores health in all its many meanings: physical, social, behavioral. Health Share is also a community of members who actively engage to the best of their abilities. Because without individual responsibility, there ultimately cannot be health. We know that it is better to prevent illness than treat it, better to stay on top of chronic conditions, better to provide people what they need to take care of themselves. In our community, members get the care they need, when they need it. What’s more, care goes into the community to serve them where they are. In our community, members receive the care they need regardless of race, ethnicity, social situation or literacy levels. Barriers are removed and doors are opened for the benefit of member health. In our community, care and services come from whatever source is most appropriate. We acknowledge, respect and include all providers of care and services. We count as partners any and all organizations that help our members, whether by contract or by mission. And that help extends far beyond the provision of care, to the basic needs that all people have and are entitled to: education, housing, employment, safety and more. Our mission is to develop an integrated community health system that achieves better care, better health and lower costs for the Medicaid population and the Tri-County community.
Established in 1910, The Urban League is the nation`s oldest and largest community- based movement devoted to empowering African Americans to enter the economic and social mainstream. Today, the National Urban League, headquartered in New York City, spearheads the non-partisan efforts of its local affiliates. There are 94 local affiliates of the National Urban League located in 35 states and the District of Columbia providing direct services to more than 2 million people nationwide through programs, advocacy and research. Springfield Township is a 6.16 square mile suburban community located along the Northwest border of the City of Philadelphia. Situated in the Southeastern part of Montgomery County, Springfield is surrounded by the Townships of Abington, Cheltenham, Upper Dublin and Whitemarsh, as well as the Chestnut Hill section of Philadelphia. According to the 2010 United States Decennial Census, Springfield Township maintains a population of 19,418 persons among 7,821 households. The majority of Springfield residents are employed in the City of Philadelphia or in neighboring suburban municipalities. The primary commercial areas are located along Bethlehem Pike in Erdenheim and Flourtown and smaller industrial pockets in Oreland and Wyndmoor. The majority of the community however is developed with single-family detached dwellings constructed during the 1940's, 50's and 60's. Springfield Township was incorporated as a First Class Township in 1901 and is further organized under the council-manager form of government. The council-manager form of government seeks to achieve the value of representation through the elected Board of Commissioners and the value of efficiency through the appointment of a professional municipal manager. The center of power in the council-manager plan is the elected Board of Commissioners, Springfield's governing body who, in turn, appoints a manager to administer the affairs of the government and supervise the service departments and agencies. Springfield Township provides a full range of municipal services including police protection, refuse and recycling collection, sewage conveyance, highway maintenance, recreational activities, programs and park areas, economic development initiatives, zoning and codes enforcement and building inspection.
