The PACE model of care provides a welcomed alternative to a nursing home PACE participants are aged 55 or older; are certified by the State of Michigan to need nursing home level of care; live in the defined PACE service area; and are able to live safely in the community, supported by PACE program services, at time of enrollment. PACE is different from other medical and social services offered in a community: PACE is a complete -- all-inclusive -- healthcare plan that is tailored to your specific needs and circumstances. A dedicated team of medical and support experts -- your Interdisciplinary Team -- works with you to design a care plan just for you. Then, PACE implements the plan and continues to monitor your health and wellness, making any necessary adjustments if and when your health changes. PACE offers you coordinated, proactive, preventive care -- individualized to you.
